Enroll

Joining the Club

Reaching out is often the hardest step, and we’re glad you’re here. We’ll walk with you through every part of enrollment, one simple step at a time. Anacortes Old Friends Club opens in 2027, and we’re building our waitlist now.

Is the Anacortes Old Friends Club right for your loved one?

The Club is for adults living with dementia, Parkinson’s, MS, stroke, brain injury or other memory and movement changes who would enjoy a day of friendship and activity. A diagnosis is not required.

Our trained team is happy to help Members with:

  • Getting around: steadying and guiding people who use a cane, walker or wheelchair
  • Standing and sitting: light, hands-on help, as long as Members can bear their own weight
  • The restroom: reminders, some hygiene help, and help with buttons and zippers
  • Mealtimes: cutting, spreading or arranging food so Members can eat on their own

We’re a social, non-medical program, so the Club may not be the right fit for someone who needs help bearing their own weight, needs to be fed, needs medication given to them, or has medical needs we can’t meet. If you’re not sure, call us (360-230-8150). We’ll talk it through together and, if the Club isn’t the right fit, help you find other options.

One easy way to get started

Download the Initial Inquiry Form, fill it in on your computer, and email it to us at foundation@pnwhomeforlife.com

How enrollment works

  1. Get in touch. Call, email, or send us the intake form. However you start, we’ll follow up to learn about your loved one and your family.
  2. Visit the Club. Bring your loved one to spend about two hours with us, arriving around 10:30, after the group has settled in. While they join the fun, you’ll meet with our team nearby to ask questions and talk through next steps.
  3. Complete the registration form. It must be finished before your loved one’s first day. You’re welcome to fill it out before your visit, or bring the details with you and we’ll help.
  4. Reserve your spot. Choose your days and pay the first month’s fee before the first day. If you need financial help, we’ll give you a short application.
  5. Start the Club! We’ll call a few days ahead so you know exactly what to expect. The first three weeks are a trial period, so everyone can make sure the Club is a good fit.

FORMS

Both forms can be filled in on your computer. For best results, open them in the free Adobe Acrobat Reader, type your answers, and save.

Please email completed forms to our secure email at foundation@pnwhomeforlife.com, or bring them to your first visit.

Along with the registration form, please bring a copy of your loved one’s POLST, if they have one. We’ll also ask you to complete a short care partner survey, which helps us support you, too.

Days, hours and cost

DaysMondays, Tuesdays and Wednesdays
Hours10:00 AM to 3:00 PM, with arrival from 9:50-10:10 AM
Monthly fee$800 for 2 days a week, $1200 for 3 days a week
IncludedLunch, snacks, activities, and a day with friends

Fees are billed at the beginning of each month and stay the same if your loved one misses an occasional day. If you start mid-month, your first month is prorated.

We believe cost should never keep a family away. Scholarships and reduced fees are available, so please ask. If your loved one has long-term care insurance, we’ll provide receipts and attendance records for reimbursement.

What to bring

  • A change of clothes in a labeled bag
  • Any personal care supplies your loved one uses
  • A sweater or rain jacket and comfortable walking shoes for garden walks
  • A packed lunch if they have a food allergy or special diet
  • A few favorite family photos for our photo album project

About medications: we’re a non-medical program, so Members who need medication during the day must take it on their own. Our team can offer reminders or open a bottle. Please talk with us about any medication needs at enrollment.

Common questions

Can my loved one come just one day a week? We recommend at least two days a week. A steady routine helps Members feel at home and build friendships, and it gives you more reliable time to rest.

Do you provide transportation? Families drop off and pick up at Wisteria Gardens.

What if my loved one’s needs change? We’ll keep you informed and work with you. If the Club can no longer meet their needs, we’ll help you find the next right step.

What happens if the weather is bad? We follow the Anacortes School District. If schools close for weather, the Club is closed too.
